To begin, select your primary location below, and you’ll be taken to the survey site. Before the questions start, you’ll see information about participating in the study and a glossary of key terms. Click “Next” at the bottom of each page to continue, and use your browser’s “Back” button if you need to go back.

To protect your confidentiality, the survey does not autosave. Please make time to finish in one sitting; if you need to step away and your device goes to sleep, you may have to start over. The survey takes about 20–30 minutes to complete.

Please note: To help us understand your range of experiences at the UW, this survey includes potentially sensitive questions about sexual harassment and sexual assault. You may skip any questions you don’t want to answer — but be sure to click “Submit” at the end of the survey so that your answers are recorded.

Look for initial results in spring 2020, with campus action plans to follow in the summer and fall.

HEALTH SCIENCES includes staff and faculty employed by one of the six health sciences schools — Dentistry, Medicine, Nursing, Pharmacy, Public Health and Social Work — as well as all students enrolled in an academic program in one of those schools and health sciences administration employees.
